Forehead lines are the lines that appear on the forehead — the horizontal creases that develop from raising your eyebrows over years of expression. They're among the most commonly treated areas in aesthetic medicine, yet also among the most commonly mismanaged.
Dynamic vs. Static Lines: A Critical Distinction
Understanding this distinction is essential before any treatment.
Dynamic lines: Appear only with facial expression (when you raise your eyebrows). In early stages, the skin returns to smooth when your face is at rest.
Static lines: Visible even at rest. Indicate that the skin has been permanently creased by years of repetitive movement.
Why it matters: Neuromodulators (Botox/Dysport) are highly effective for dynamic lines by reducing the muscle movement causing them. For static lines, some improvement is possible, but surface treatments (microneedling, resurfacing) may be needed to address the actual skin crease.
Why Overly Frozen Foreheads Happen

The frontalis muscle is responsible for brow elevation. Over-treating the forehead can drop the brows significantly, creating a heavy, aged appearance — the opposite of the intended effect.
The goal is softening, not eliminating. Patients should maintain some forehead movement. Complete elimination of movement rarely looks natural.
Individual assessment is critical. Forehead length, brow position, skin quality, and expression habits all influence how much product is appropriate.
A Nuanced Treatment Approach

- Assess brow position first — patients with already-low brows need very conservative treatment to avoid brow ptosis
- Treat conservatively — less product, see the result, add more if needed
- Address the "11s" (glabellar lines) together with the forehead — treating these depressor muscles allows brow elevation to offset any brow-lowering effect
- Consider surface treatments — for established static lines, microneedling or resurfacing may be needed alongside neuromodulators
