Marionette Lines: Causes & Treatment Options
Marionette lines are the vertical creases that run from the corners of the mouth down toward the jaw — named after the hinged mouth of a puppet. Even on a happy, rested face, deep marionette lines can read as tired or downturned, which is why they're one of the most common concerns we hear about.
What Causes Marionette Lines
They form from a combination of factors that add up over time:
- •Volume loss. As the fat pads and bone of the lower face change with age, the skin loses its support and begins to fold.
- •Skin laxity. Collagen and elastin decline, so skin no longer bounces back.
- •Gravity and movement. Years of expression and the steady pull of gravity deepen the crease.
- •Genetics and lifestyle. Sun exposure, smoking, and family patterns all play a role.
Non-Surgical Treatment Options
The right approach depends on whether your lines come mostly from volume loss, skin laxity, or muscle pull — which is exactly what a consultation sorts out.
- •Dermal filler is the workhorse for marionette lines. Placed strategically along the fold and to restore chin and jaw support, it lifts the crease and rebalances the lower face. Often the most immediate, visible improvement.
- •Biostimulators (such as poly-L-lactic acid or polynucleotides) rebuild collagen gradually for longer-term skin quality and support — a good complement to filler.
- •Neuromodulators (Botox). A small amount can relax the muscle that pulls the corners of the mouth downward (the DAO), softening the "downturned" look. Usually a supporting role rather than the main fix.
