Under-Eye Hollows: Filler vs. Alternative Treatments

Under-eye hollows — technically called "tear troughs" — are one of the most common concerns patients bring to our practice. The shadowy, sunken appearance can make you look exhausted even after a full night's sleep. But treating this delicate area requires careful consideration.

Understanding Under-Eye Anatomy

The under-eye area is complex:

  • Skin is extremely thin — the thinnest on the face
  • There's minimal fat padding — making the transition from eyelid to cheek visible
  • Orbicularis muscle sits just beneath the skin
  • Orbital bone creates the foundation

Hollow appearance can result from: - Loss of fat in the tear trough - Descent of mid-face volume (making the hollow more prominent by contrast) - Bone remodeling that deepens the socket - Thin, translucent skin that shows underlying structures

When Tear Trough Filler Works Well

Filler can be an excellent option when:

  • The primary issue is volume loss — actual hollowing rather than other concerns
  • Skin quality is good — thick enough to camouflage filler
  • No significant fat pads — puffy "bags" can be worsened by filler
  • Patient understands limitations — filler improves, not perfects

Dr. Taborga uses very conservative amounts of soft, smooth filler placed deep on the bone. The goal is subtle improvement, not complete elimination of the shadow.

When Filler May Not Be the Best Choice

Eye bags (fat prolapse): If you have puffy bags, adding filler below them can make them more prominent. Surgery (blepharoplasty) may be more appropriate.

Significant skin laxity: Very thin, crepey skin won't camouflage filler well and may develop visible lumps or a bluish tint (Tyndall effect).

History of filler migration: Some patients' anatomy causes filler to migrate or accumulate over time, creating the "filler fatigue" look.

Unrealistic expectations: If you want complete elimination of all shadows, you'll likely be disappointed with any treatment.

Alternative Approaches

Mid-face filler: Sometimes the best treatment for under-eyes is... not treating under-eyes directly. Restoring cheek volume can lift the entire area and reduce tear trough depth indirectly.

PRP therapy: Platelet-rich plasma can improve skin quality and stimulate collagen in the under-eye area without adding volume.

Skincare: Retinoids, vitamin C, and peptides can improve skin thickness and quality over time.

Surgery: Lower blepharoplasty remains the gold standard for significant bags or excess skin.
