Treating Smoker's Lines (Perioral Lines)

6 min readBy

Smoker's lines, more accurately called perioral lines, are the fine vertical wrinkles that form above the upper lip. They develop with age, sun exposure, repetitive lip movement, and yes, smoking, though many non-smokers get them too. They can be softened with a combination of conservative filler, biostimulators, and good prevention, individualized to your skin.

Why perioral lines form

The skin around the mouth is thin and constantly moving as you speak, eat, and express. Over time, collagen and elastin decline, and repeated pursing of the lips etches lines into the surface. Sun damage accelerates the process, and smoking compounds it through both the repetitive motion and its effect on skin and circulation. The result is the familiar pattern of vertical creases radiating from the lip border.

Treatment options

There is no single fix; a physician often blends approaches based on the depth of the lines and your skin quality. Common, conservative options include the following.

  • Soft hyaluronic acid filler placed delicately to plump individual lines and restore lip border support
  • Biostimulators such as PDRN and polynucleotides to support skin quality over time
  • Neuromodulators (Botox or Dysport) used very conservatively to soften the muscle pull that deepens lines
  • Skin-quality measures including diligent sun protection and a sensible topical routine

Why a light touch matters

The perioral area is delicate and prone to looking unnatural if overtreated. Heavy filler here can blunt the lip or migrate above the border, and overly aggressive neuromodulator can affect the way you speak, smile, or drink from a straw. A physician treats this zone with restraint, often combining small amounts of more than one approach rather than overloading any single one. The goal is softer lines, not a stiff or altered mouth. Deep, long-standing lines may soften rather than disappear entirely, and a physician will set that expectation honestly rather than promising a perfectly smooth upper lip.

Prevention going forward

Results last longer when you address the factors that created the lines in the first place. Daily sun protection, not smoking, and good skin hydration all help slow new lines from forming. Your physician can fold these habits into a realistic plan so that treatment is supported by prevention rather than constantly working against ongoing damage. Maintaining your result is partly about daily care, not just in-office treatment.

What to expect

Treatment is typically done in office with numbing for comfort. Mild swelling or bruising may occur in this sensitive area and usually settles within days. Filler results are visible relatively soon, while biostimulator and skin-quality results build gradually over several weeks. Suitability for each option, and the right combination for you, is always determined at consultation. Results vary by individual, and the plan is built around your face.
